System Crashes to Black Screen with Diagonal White Lines

Problem: The Toaster software is operating normally, when suddenly the system crashes, and the output video becomes a black screen with diagonal "dashed" white lines. If you reboot the system, when you run the Video Toaster software, the system crashes, with the same resulting output on video.
